The 2019 PIDA submission and criteria forms can be downloaded as a “fillable” PDF forms below. (we recommend using the latest version of Adobe’s FREE Acrobat Reader which can be downloaded here.)

Step 1: Save a copy of the PDF registration form to your computer.
Step 2: Open the PDF file that you downloaded to your computer and complete your details in the fields provided.
Step 3: IMPORTANT – To save the completed form, choose File > ‘Save As’ and rename the file so that it has a different file name than the original form. (If you only ‘save’ the file your text may be lost).
